You could use a pillow case (as the title suggests) or cut a piece of fabric to your desired length, spread it out, sew the right side vertical edges together and hem the bottom. Just remember to leave enough space at the top for "arm holes"!
Once you've sewn a "sheath" (approximately 1 inch wide) at the top of the front and back pieces, you can insert a length of ribbon or fabric cord to form the "neck hole". I fashioned a coordinating spaghetti-width strap of the same material as the dress. Just be sure to make the strap/ribbon/cord long enough to tie into a bow--PLUS make sure to tie it securely! Otherwise, the entire dress will simply slide right off if you're not careful (not speaking from experience or anything, of course). :)

Because she's not much of a "lady" yet on the playground, I made her some coordinating "modesty shorts" as well. Now when she goes down the slide and does somersaults, she's covered. Every little girl, who likes to wear dresses but also likes to be a tomboy, should have a bunch of these shorts on hand!
